1. Nature of Request
Submitting this form constitutes a request for surrender, not a guarantee of acceptance. Safescape
Foundation reserves the right to accept or decline any surrender request at its sole discretion.
2. Ownership / Caregiver Declaration
The Applicant confirms that:
a. They are the legal owner of the pet; or
b. They are the primary caregiver/rescuer of the pet and have lawful possession and authority to request surrender; or
c. They have obtained consent from the legal owner or relevant authority to initiate this surrender.
The Applicant agrees to provide valid identification and any supporting proof (if available) upon
request. The Organization reserves the right to verify the Applicant’s claim before accepting the surrender.
3. Reason for Surrender
The Applicant must provide truthful and complete information regarding the reason for surrender.
Misrepresentation or concealment of facts may lead to rejection of the request.
4. Health & Behaviour Disclosure
The Applicant agrees to disclose all known details about the pet, including:
a. Medical history, vaccinations, and ongoing treatments
b. Behavioural traits, including aggression, anxiety, or bite history
Failure to disclose critical information may endanger others and will be treated as a serious violation.
5. Evaluation & Acceptance
Safescape Foundation may:
a. Conduct screening calls or visits
b. Request additional information or documentation
c. Decline intake based on capacity, resources, or suitability
Acceptance of surrender is subject to availability and welfare considerations.
6. Transfer of Custody
Upon acceptance, the Applicant agrees to transfer full custody of the pet to Safescape Foundation. The
Applicant relinquishes all ownership rights and claims over the pet thereafter.
7. No Reclaim Policy
Once the pet is surrendered and accepted, the Applicant shall have no right to reclaim the pet, unless
expressly permitted by the Organization at its discretion.
8. Financial Contribution (if applicable)
Any contribution made by the Applicant towards the pet’s medical care, boarding, or rehabilitation is voluntary.
9. Welfare Commitment
The Applicant acknowledges that the Organization will act in the best interest of the animal’s welfare,
which may include medical treatment, foster care, or adoption placement as deemed appropriate.
10. Urgency Misuse
The Applicant acknowledges that surrender requests marked as urgent without valid justification may be
deprioritized or declined. The Organization reserves the right to assess urgency based on the pet’s
welfare and available capacity.
11. Consent for Communication
The Applicant consents to being contacted by Safescape Foundation for follow-ups, clarifications, or
updates regarding the surrender request.
